The BAPP member agreement (downloadable below) has several parts as follows:

• Pages 1-5 comprise the contract itself.  Please sign and send the signature page to BAPP.
• Addendum A, pages 8-29 comprise the credentialing application.  We use the CMA-approved participating physician agreement.  If you have a current credentialing application on this form, you may submit a copy rather than copying the information onto our form.
• Addenda B-D explain our operating procedures
